Vietnam: 14 forms used in the procedures for benefits from the Occupational Accident and Occupational Disease Insurance Fund

Issued together with Decree No. 88/2020/ND-CP of Vietnam’s Government are 14 forms used in the procedures for assistance from the Occupational Accident and Occupational Disease Insurance Fund (hereinafter referred to as “the Insurance Fund”, specifically as follows:

- Form No. 01: Application for occupational disease benefits;

- Form No. 02: Application for assistance for costs of occupational disease examination or treatment;

- Form No. 03: Application for financial assistance in career change training;

- Form No. 04: Decision on provision of financial assistance in career change training;

- Form No. 05: Application for financial assistance in occupational disease examination;

- Form No. 06: Decision on provision of financial assistance in occupational disease examination;

- Form No. 07: Application for financial assistance in occupational disease treatment;

- Form No. 08: Decision on provision of financial assistance in occupational disease treatment;

- Form No. 09: Application for financial assistance in occupational rehabilitation;

- Form No. 10: Decision on provision of financial assistance in occupational rehabilitation;

- Form No. 11: Application for financial assistance in occupational safety and health training;

- Form No. 12: Decision on provision of financial assistance in occupational safety and health training;

- Form No. 13: Plan on financial assistance in risk prevention and sharing with respect to occupational accidents and diseases and expenses for management of occupational accident and disease insurance;
