Decision 62/QD-BYT of 2023 announcing administrative procedures to be amended and supplemented in the field of health insurance under the management functions of the Ministry of Health of Vietnam as prescribed in Decree 104/2022/ND-CP amending the Decree related to the submission and presentation of household registration books and paper temporary residence books when performing administrative procedures and providing public services.

Procedure for renewing the health insurance card in Vietnam

Step 1:

Participants in health insurance need to exchange their health insurance cards with the social insurance agency of the province or district; write the declaration of participation and adjustment of health insurance information issued by the Vietnam Social Insurance; submit the one-stop section to the provincial and district social insurance agencies; wait for signature (in the box of the applicant) in the application receipt; and make an appointment to return the results of the re-issuance of the health insurance card according to Form No. 4 of the Appendix issued together with the Decree 146/2018/ND-CP as amended in Decree 104/2022/ND-CP.

Step 2:

- An officer of the one-stop section of the social insurance agency of the province or district shall check and receive the declaration of participation, adjust health insurance information, record the receipt of the dossier, and make an appointment to return the result of the change of the health insurance card according to Form No. 4 of the Appendix issued together with Decree 146/2018/ND-CP as amended in Decree 104/2022/ND-CP and signed (in the box of the person receiving the application and giving the Form 4 to the health insurance participant to sign in the box of the person submitting the application).

- Within 7 working days from the date of receipt of complete dossiers. The health insurance organization must change the health insurance card for the insured.

Components of a dossier for changing health insurance cards include:

The health insurance card is changed in the following cases:

While waiting for the card to be changed, the participants in health insurance will still be entitled to health insurance benefits.

Instructions on filling out Form No. 4

1. The receipt of the application and the appointment to return the results are made in 02 copies, one for the individual or organization submitting the application, the other with the same file for the Operations Department and then kept at the Receiving Department. and return the results.

2. An application receipt and an appointment to return results are made for each type of application according to each administrative procedure (for example, a unit that submits 3 different types of documents will have 3 receipts and an appointment to receive the results). returns results).

3. In the content of the request for settlement: write a summary of the request for settlement of administrative procedures.

Some cases to note:

- In case an individual requests to re-issue, merge, change, or adjust the information recorded on the health insurance card: the officer of the Department receives the dossier and returns the results, fully recording the contents that the individual requests to be resolved; at the same time, write down the old health insurance card code to use the appointment slip to replace the health insurance card when going for medical examination and treatment;

- In case the unit requests to re-issue, merge, change, or adjust the information recorded on the health insurance card: Officers of the Department receive dossiers and return the results of checking the completeness and validity of the dossiers, enclosed with the declaration of supply and change of information on health insurance participants.

- In case the health insurance participants applying for the Certificate of Co-payment do not have the same payment period during the year, they have participated in health insurance in many different places: Specify the name of the unit participating in health insurance and social insurance in the province/city where health insurance has been paid.

4. Individuals register to receive results directly at the social insurance agency, when coming to receive the result as the payment for the health insurance regime, the department officer receives the dossier, returns the results, and instructs the individual as follows:

- Direct recipients: provide an appointment letter and citizen ID card or identity card.

- Others accept:

+ If you are a relative of the beneficiary: provide an appointment letter, an ID card, or citizen identification card, and one of the following papers proving that you are a relative of the beneficiary of health insurance: copy of the birth certificate or Marriage certificate or Certificate of residence information, or Notice of personal identification number and citizen information in the National Population Database.

+ If you are a guardian: provide an appointment letter, an ID card or citizen ID card, and documents proving that you are the natural guardian of the beneficiary of health insurance (copy of birth certificate, marriage certificate, certificate of residence information or Notice of personal identification number and citizen information in the National Population Database) .

In case there is no natural guardian as prescribed by law, an appointment letter, a person's identity card or citizen identification card, and a decision on the recognition of guardianship shall be provided by the competent authority.

+ If they are not relatives or guardians mentioned above: provide an appointment letter, an identity card or citizen identification card, and a power of attorney in accordance with current law./.
