Notification on Adjustment of 07 Electronic Document Transaction Procedures

According to Notification 2459/TB-BHXH, Ho Chi Minh City Social Insurance has adjusted and supplemented the electronic dossier reception procedures, effective from October 14, 2016, as follows:

  1. Procedure for Adjusting Personal Information of Participants in Social Insurance (Procedure and Electronic Dossier Receipt Form 302);
  2. Procedure for Adjusting Personal Information Borrowed From Another Person's Dossier for Social Insurance Participation (Procedure and Electronic Dossier Receipt Form 303);
  3. Procedure for Consolidating Social Insurance Participation Records from Multiple Books to the Original Book (Procedure and Electronic Dossier Receipt Form 304);
  4. Procedure for Reissuing Lost Social Insurance Books: due to loss by the employee or loss by the unit; recovering lost Social Insurance books that were found (Procedure and Electronic Dossier Receipt Form 305);
  5. Procedure for Reissuing Damaged Social Insurance Books (Procedure and Electronic Dossier Receipt Form 306);
  6. Procedure for Adjusting the Contents Recorded in the Social Insurance Book when the Employee has Stopped Contributing and the Social Insurance Book was Finalized (Procedure and Electronic Dossier Receipt Form 311);
  7. Procedure for Reissuing or Replacing Damaged Health Insurance Cards, Adjusting Benefits, Changing Medical Examination and Treatment Places, or Changing the Continuation Period of 5 Consecutive Years (Procedure and Electronic Dossier Receipt Form 402).

Additionally, in the Dossier Receipt Forms (PGNHS), six forms are adjusted: PGNHS 302, PGNHS 303, PGNHS 304, PGNHS 305, PGNHS 306, and PGNHS 402.

The process of consolidating Social Insurance participation records from multiple books to the original book in cases where one or more Social Insurance books are being held at the Social Insurance agency is also abolished (Procedure 315).

The adjustment procedures are summarized in the following diagram:

  1. Procedure for Adjusting Personal Information of Participants in Social Insurance. Electronic Dossier Receipt Form 302.

  2. Procedure for Adjusting Personal Information Borrowed from Another Person's Dossier for Social Insurance Participation. Electronic Dossier Receipt Form 303.

  3. Procedure for Consolidating Social Insurance Participation Records from Multiple Books to the Original Book or in cases where one or more Social Insurance books are being held at the Social Insurance agency. Electronic Dossier Receipt Form 304.

  1. Procedure for Reissuing Lost Social Insurance Books: due to loss by the employee or unit; recovering lost Social Insurance books that were found (Electronic Dossier Receipt Form 305).

4.1. Procedure for Recovering Lost Social Insurance Books that were Found: by the employee or by the unit.

  1. Procedure for Reissuing Damaged Social Insurance Books (Electronic Dossier Receipt Form 306).

  1. Procedure for Adjusting Contents Recorded in the Social Insurance Book when the Employee has Stopped Contributing (Electronic Dossier Receipt Form 311).

  1. Procedure for Reissuing or Replacing Damaged Health Insurance Cards, Adjusting Benefits, Changing Medical Examination and Treatment Places, or Changing the Continuation Period of 5 Consecutive Years (Electronic Dossier Receipt Form 402).
