- Based on the participant's health insurance card code, look up the information on card validity in the Online Lookup section / Health Insurance Card Validity Lookup on the Vietnam Social Security website (http://baohiemxahoi.gov.vn). If at the time of registration, compared to the old card's validity, it is not more than 03 months in the financial year, then extend the validity. If it exceeds 3 months, create a new registration. - Contact the district Social Insurance Agency where the agency is headquartered to obtain a management code and receipt (Form C68-HD), organize the collection of money from registrants of voluntary social insurance, health insurance for urban households residing in the city (regardless of district), compile and submit documents via the electronic transaction system. - collectors that received the receipt (Form C68-HD) according to Official Dispatch 1055/BHXH-KHTC dated May 26, 2017, should contact the district Social Insurance Agency that issued the receipt to complete the return procedure. Note: Participants who are the second person or more in the family-based health insurance will receive a reduced contribution rate for the financial year. |

Provide USER (authority to adjust personal information and print family-based health insurance cards) to the Director of the district Social Insurance Agency for arranging the health insurance card exchange issued by other district Social Insurance Agencies. Periodically, summarize the status of health insurance card exchanges from the USER to provide to the ID Card Issuance Department for monitoring and managing card forms. |
