Dossier and Procedures for Adjusting Personal Information on the Social Insurance Book

Currently, there are many cases where incorrect personal information (Name; Date of birth; Gender; Nationality; Ethnicity) of employees is provided during the process of declaring records for participation in Social Insurance (BHXH) or during record processing, resulting in incorrect information printed on the Social Insurance book. Therefore, when receiving the Social Insurance book for storage or processing Social Insurance policies, the entity or employee needs to prepare the documents to correct the personal information accurately to avoid affecting future Social Insurance policies.

Clause 2 Article 27, Clause 2 Article 29 Decision 595/QD-BHXH of 2017 stipulates the dossier and procedures for reissuing the social insurance book due to changes in surname, name, middle name; date, month, year of birth; gender, ethnicity; nationality; adjustment of the contents recorded in the social insurance book as follows:

1. Subjects for dossier preparation:

- If the employee is currently working at the unit, the current unit shall submit the adjustment dossier.

- If the employee has retired, they may submit the adjustment dossier themselves to the former managing social insurance agency or where they are permanently/temporarily residing.

2. Components of the dossier

In case the unit declares the dossier:

- Social insurance book;- Form TK1-TS;- Form D01-TS (For the case where the unit prepares the dossier for reissuing the social insurance book);- Documents serving as a basis for adjusting incorrect personal information (Birth certificate or birth extract issued by the competent authority in charge of civil status according to regulations and identity card/citizen identity card/passport).

In case the employee declares the dossier themselves:

- Social insurance book;- Form TK1-TS;- Documents serving as a basis for adjusting incorrect personal information (Birth certificate or birth extract issued by the competent authority in charge of civil status according to regulations and identity card/citizen identity card/passport).

Number of dossiers: 01 set.

3. Processing time

No more than 10 days from the date of receiving the complete dossier as prescribed. In cases where it is necessary to verify the social insurance contribution process in another province or multiple units where the employee has worked, not exceeding 45 days but a written notice must be issued to inform the employee.
