Compilation of Forms for Enterprises Requesting Temporary Suspension of Retirement and Survivorship Fund Contributions

Thu Ky Luat respectfully sends to our valued clients, who are enterprises and business households eligible to temporarily suspend contributions to the retirement and survivorship fund, the following forms to be used in the request dossier:
Report on the number of employees stopped working or losing jobs due to the impact of the Covid-19 pandemic at enterprises, business households, and production facilities.
Official Dispatch requesting confirmation of conditions to temporarily suspend compulsory social insurance contributions.
List of employees at the time before suspending production and business and at the time of request.
List of employees participating in social insurance who must temporarily stop working.
Note: In addition to the dossier (original copies) of the above documents, enterprises are responsible for providing the following comparative documents:
- Business registration certificate; documents related to the enterprise's temporary suspension of production and business due to the Covid-19 pandemic;- Timesheets and payroll list at the time before suspending production and business and at the time of the official dispatch request;- Documents related to temporarily stopped employees (agreement on temporary labor contract suspension, agreement on leave without pay, work stoppage, etc.);- Monthly reports on social insurance, health insurance, unemployment insurance, occupational accident and disease insurance contributions issued by the Social Insurance agency (most recent contribution period results).
