In addition, Appendix II regulations on Forms and instructions for recording commune-level specialized population statistics reports (Issued together with Circular 01/2022/TT-BYT effective March 1, 2022) also provides guidance on preparing monthly specialized population statistics reports at commune level (Form 01-DSX) as follows:
1. Purpose: Reflects the number of children born; number of pregnant women and results of contraceptive service provision in the area during the month.
2. Way to synthesize and take notes:
Write the name of the place (in CAPITAL LETTERS) on the upper right side of the chart
Enter the report document number and the month and year of the report
For example:
Report September 2022 Nguyen Hoa commune, Phu Cu district, Hung Yen province.
Note: Report September 2022, report receipt date is October 5, 2022

Column 1. Data of the entire commune: Add data of villages, hamlets, and residential groups in the commune level (add data columns).
From column 2 onwards: Record the names of villages, hamlets, and residential groups in the commune level.
In case a village has many areas (each population collaborator manages one area), the village column is divided into several columns including the addition column and the area name columns. The village's data will be added to the data of the population collaborator's areas.
The following lines are summarized and recorded as follows:
(1) Total number of children born in the month: Add up the data in Section 1 of Form P0/CTV
- Number of male children born: Add the number of children born in the date of birth column and the gender is male.
- Number of female children born: Add the number of children born in the date of birth column and the gender is female.
- The number of children born as the second child: Add the number of children born as "the second child" in the column "what is the mother's second child".
- Number of children born as the 3rd child or more: Add the number of children born as "the 3rd child" or more in the column "what is the mother's third child".
- Number of children born to mothers under 20 years old: Add the number of children born to mothers under 20 years old in the "mother's date of birth" column.
(2) Number of pregnant women during the month: Add the number of pregnant women at the end of the month in Section 8.
(3) Number of women with new IUDs in the month: Add the number of women with IUDs in Section 6 and write Number of women with IUDs replaced.
(4) Number of women who stopped using IUDs during the month: Add the number who stopped using IUDs in Section 7.
(5) Number of newly sterilized men in the month: Add the number of newly sterilized men in Section 6.
(6) Number of newly sterilized women in the month: Add the number of newly sterilized women in Section 6.
(7) Number of new female contraceptive implants in the month: Add the number of new female contraceptive implants in Section 6 and record the number of contraceptive implant replacements.
(8) Number of women who stopped using contraceptive implants during the month: Add the number who stopped using contraceptive implants in Section 7.
3. Data source: Compiled from Form P0/CTV reviewed and appraised by the entire commune.
