Pursuant to Appendix II stipulating the Form and instructions for recording the commune-level statistical reports on population (issued together with the Circular 01/2022/TT-BYT effective on March 1, 2022), the form of statistics reports by quarter of commune-level in population (Form 02-DSX) is as follows:

Instructions for making statistics reports by quarter of commune-level in population in Vietnam

In addition, Appendix II stipulating the Form and instructions for recording the commune-level statistical reports on population (issued together with the Circular 01/2022/TT-BYT effective on March 1, 2022) also provides guidance on the preparation of statistics reports by quarter of commune-level in population (Form 02-DSX) as follows:

1. Purpose: Reflection of the population situation, population change, family planning, results of providing services to improve population quality, and periodical health check-ups for the elderly in the commune. Evaluation of the propaganda and advocacy work on population, human resources to carry out population work.

2. Way to summarize and record:

Enter the name of the place (in capitals ONLY) on the top right of the form; quarterly report.

Column 1. Data for the whole commune: The data is obtained by adding the data of the villages in the commune (plus the data columns) and the commune data provided by the health facility.

From column 2 onwards, write the names of villages in the commune. Some lines only have commune data.

a) The following lines are compiled from the P0/CTV slip, A0/CTV slip, and A0 book

(1) Total number of households (collective households + households) at the end of the quarter: Add the number of households and collective households in Book A0; Record the number of households.

(2) Total number of actual permanent residents (population) at the end of the quarter: Add the actual number of permanent residents in Book A0.

(3) Total number of married women aged 15-49 at the end of the quarter: Add the actual number of permanent residents who are women aged 15 to 49 years (according to the calendar year) and whose marital status is "married" in Book A0.

(4) Number of deaths in the quarter: Add the data in Section 2 of the P0/CTV form.

(5) Number of people getting married in the quarter: Add the number of people getting married (changing marital status) in Section 5 of the P0/CTV slip.

- Number of people getting married: Add the number of married men under 20 years old and married women under 18 years old in Section 5 of the P0/CTV form.

- Number of people who are married by blood: Add the number of people who are married and have blood relations within 3 generations as prescribed by law in Section 5 of the P0/CTV form.

(6) Number of married couples who have received pre-marital counseling and medical examination in the quarter: Add the number of married couples that both parties have received pre-marital counseling and medical examination in the quarter in Section 5 of the Voucher. P0/CTV.

(7) Number of people divorced in the quarter: Add the number of people divorced (change in marital status) in the quarter in Section 5 of the P0/CTV slip.

(8) Number of people moving from another commune during the quarter: Add up the number of people moving from another commune, including the person(s) who moved to the whole household and not including the person(s) who moved within the commune during the quarter in Section 3 of the P0/CTV Voucher.

(9) Number of people who moved out of the commune during the quarter: Add the number of people who moved out of the commune, including those who moved out of the household and excluding the person(s) who moved within the commune during the quarter in Section 4 of the P0/ CTV.

(10) Number of couples of childbearing age currently using contraception at the end of the quarter: Add up the number of married women aged 15-49 years who are currently using contraception (which is condoms) at the end of the quarter and each condom at the end of the quarter. Section II in the Form A0/CTV.

(11) Number of couples of childbearing age who have not used contraception at the end of the quarter: Add the number of married women aged 15-49 years who are not currently using contraception (code 0) as of the end of the quarter in Section II in the form A0/CTV.

b) The following lines are compiled from information on health facilities and commune health stations providing services. These lines only record data for the whole commune, no data for villages.

(12) Number of people who have consulted and examined health before marriage.

(13) Number of pregnant women who were screened and screened before giving birth for 4 diseases ( Down syndrome, Edwad syndrome, Patau syndrome, Thalassemia).

(14) Number of pregnant women screened, prenatal screening for 3 diseases (Down syndrome, Edwad syndrome, Patau syndrome).

(15) Number of newborns who were screened and screened for all five diseases (congenital hypothyroidism, G6PD deficiency, congenital adrenal hyperplasia, congenital hearing loss, congenital heart disease).

(16) Number of newborns screened, newborn screening for 3 diseases (congenital hypothyroidism, G6PD deficiency, congenital adrenal hyperplasia).

(17) Number of elderly people receiving periodical health check-ups by health stations.

c) The following lines are compiled from Book A11/TYT and directly performed by population collaborators, commune population officers, focal units doing commune-level population work.

(18) Number of households receiving counseling and propaganda about population directly from the collaborator.

(19) Number of times of propaganda on population work in the commune.

(20) Number of population collaborators as of the end of the quarter.

(21) Number of villages without population collaborators at the end of the quarter.

3. Data source: Processed from the specialized population data warehouse, P0/CTV form, A0/CTV form, A0 book and other information to make reports.
