Plan for Responding to Incidents at Facilities Using, Storing, Packaging, and Transporting Radioactive Sources of Group 3, Group 4
Section I, Part II, Appendix II on Security Requirements for Radioactive Sources and the Transport of Nuclear Materials issued with Decree 142/2020/ND-CP stipulates the plan for responding to incidents at facilities using, storing, packaging, and transporting radioactive sources of Group 3, Group 4 as follows:
Legal basis, scope of adjustment, and subjects of application of the incident response plan; explanation of concepts and terms used in the incident response plan.
Description of the type of radiation work of the facility; analysis of risks and identification of potential incident situations and consequences.
Response scenarios for each incident situation as described in Item 2, including the following contents:
a) Receiving and processing incident information, notifying participating organizations and individuals in the incident response;
b) Response procedures for each incident situation;
c) Criteria for deciding to end the incident response;
d) Remediation of environmental consequences, monitoring and health treatment for victims (if any);
e) Incident termination report: Recipients, time of sending, content of the report (incident response activities; remediation measures taken, consequences for humans and environment; radiation dose assessment, conclusion, and recommendations).

Based on Item 5, Section I, Part II, Appendix II on Security Requirements for Radioactive Sources and the Transport of Nuclear Materials issued with Decree 142/2020/ND-CP stipulates the attached documents to the incident response plan for facilities using, storing, packaging, and transporting radioactive sources of Group 3, Group 4 as follows:
- Attached documents:
a) List of radioactive sources, radiation equipment of the facility (Source/device information, relevant licenses);
b) Layout diagram of the facility, where radiation work is conducted;
c) List of equipment used in incident response;
d) List, addresses, contact phone numbers of organizations and individuals participating in the incident response and supporting organizations and individuals;
e) Forms: Notification and receipt of information; request for assistance; report during and after incident response.
