[01]. Household head information (applicable to the case of multiple members): Declare correctly the Citizen Identification Number or Personal Identification Number, full name, date of birth, gender of the household head family.

[02]. Participants in health insurance supported by the state budget: Select exactly one of the three beneficiaries of health insurance that are supported by the state budget, specifically:

- Subjects belonging to near-poor households according to the near-poor criteria as prescribed by the Government and the Prime Minister.

- Subjects of multidimensionally poor households that do not fall into the cases specified at Point a, Clause 9, Article 3 of Decree 146/2018/ND-CP dated October 17, 2018.

- Subjects of households engaged in agriculture, forestry, fishery and salt production with an average standard of living as prescribed by the Government or the Prime Minister.

[03]. Information about family members participating in health insurance with state budget support:

[03.1]. citizen identification/personal identification Number: Accurately declare the participant's Citizen Identification Number or Personal Identification Number.

[03.2], [03.3], [03.4], [03.5]. Full name, social insurance number, date of birth, gender: The system automatically displays the participant's information according to the declared citizen identification/personal identification number.

[03.6], [03.7], [03.8). Address to receive results, phone number, email address: The system automatically displays information in the management database according to the declared citizen identification/personal identification number. In case of changes, participants edit and update.

[03.9]. Number of months of enrollment in health insurance: Register the number of months of participation in health insurance according to the time limit specified in the document approved by a competent authority and a list of one of the three beneficiaries of health insurance that are supported by the state budget.

[03.10]. Place of initial medical examination and treatment: Select the place to register for initial medical examination and treatment according to the list of establishments registered for initial medical examination and treatment automatically displayed by the software system.

[03.11]. Register to receive health insurance card:

- Electronic health insurance card and health insurance card image on VssID application and use citizen identification card for medical examination and treatment instead of health insurance card.

- Paper health insurance card for additional registration: Participants register to receive a paper health insurance card (in addition to the fact that the social insurance agency has already paid the electronic health insurance card and the image of the health insurance card on the VssID app and uses the citizen identification card to go for medical care replacing health insurance cards). Participants choose to receive health insurance cards in one of two forms:

+ Pay at the social insurance agency: The software system automatically displays the address of the social insurance agency where the application is received so that the participants know and come to receive the health insurance card.

+ Pay via postal service (with fee): Participants fully and accurately declare the address to receive results for the postal service to return results.

[04]. Unit code of the service organization collecting social insurance and health insurance to mobilize and advise (if any): Declare the unit code (granted by the social insurance agency) of the service organization that collects social insurance and health insurance to be provided when campaigning and consulting. 

After declaring complete and accurate information. Participants confirm and take responsibility for the information declared on the Public Service Portal.
