Currently, there are no regulations on general notarization request forms. However, the Law on Notarization 2014 has regulations on the information required in the form including:
- Information about the full name, address, and phone number of the person requesting notarization
- Notarization request content;
- List of attached documents;
- Signature of the person making the request for notarization.
Due to the lack of specific regulations, individuals or organizations in need of notarization can prepare their own templates but still need to have all the necessary information as mentioned above, or they can directly visit a notary office or notary department where they will carry out the procedures to request this template.
When requesting notarization, you need to present documents such as:
- Form requesting notarization of contracts and transactions;
- Draft contracts and transactions;
- Copy of identification documents;
- Copies of documents proving property rights or documents related to that property;
- Copies of other documents related to contracts and transactions that are required by law.
Note that when submitting a copy, the person requesting notarization must present the original for comparison.
Individuals and organizations that notarize contracts proceed with the following steps:
Step 1: Prepare and submit documents to the notary office:
– The person requesting notarization needs to prepare a complete dossier, including the prescribed documents, in which the contract notarization request form is an indispensable document, and submit it to the notary office or the notary office where it is most convenient.
– After receiving the dossier, the notary will check the notarization request dossier. If the dossier is complete according to the law, it will be accepted and recorded in the notary book. At that time, the dossier components are the basis for comparison, review, and legality checks.
Step 2: The notary performs the operation and notarizes the contract:
When receiving a notarization request, the notary will check the documents:
- If the dossier is complete and legal, it will be accepted and recorded in the notary book.

The notary instructs the notarization requester to comply with the regulations on notarization procedures. Explain to the notarization requester their legal rights, obligations, and interests, as well as the meaning and legal consequences of participating in contracts and transactions.
Step 3: Draft and sign the document
+ In case the document has been prepared by the notarization requester: The notary public shall examine the draft document, and if it contains provisions that violate the law, go against social ethics, or the content of the document is not in accordance with legal regulations, the notary public must clearly indicate to the requester the need for necessary revisions. If the requester does not make the required revisions, the notary public has the right to refuse notarization.
+ In case the document is drafted by a Notary at the request of the notarization requester: if the content and intention to enter into a contract or transaction are authentic, do not violate the law, and are not contrary to social ethics, the Notary shall draft the contract or transaction;
+ The notarization requester reads the draft contract himself, or the notary public reads it to the notarization requester according to the notarization requester's request. In case the notarization requester requests an amendment or supplement, the notary will consider and make the amendment or supplement on the same day or reschedule;
+ In case the notarization requester agrees with all the contents stated in the draft contract, the Notary shall guide the notarization requester to sign each page of the contract.
Step 4: Sign the certificate
The notary requires the person requesting notarization to present the originals of the prescribed documents for comparison before recording the testimony, signing each page of the contract, and transferring it to the fee collection department of the notary practice organization.
Step 5: Return notarized results
The fee collection department of the Notary Office completes the collection of fees, notary remunerations, and other expenses according to regulations, stamps and returns the documents to the notarization requester.
