Below are the guidelines to detail the personal review form: Members of Vietnam Communist Party not holding leadership or management roles can refer to the following guidelines:
* Fill in basic information:
Full name; date, month, year of birth; positions within the Communist Party and governmental, organizational roles; work unit and name of the Party cell.
* Content of personal review
I. Strengths and results achieved
[1] Regarding political qualities, ethics, lifestyle; spirit of cohesion and unity within the organization; discipline, responsibility for setting an example, adherence to what party members are not allowed to do; work habits and style. Connect with manifestations of degeneration, "self-evolution", "self-transformation".
Possible factors to consider:
- Political ideology: Loyalty to Marxism-Leninism, Ho Chi Minh's ideology, and the Communist Party's reform direction; compliance, advocacy, and persuasion of family and citizens to implement the Communist Party's directives, state policies, and laws; engagement in learning resolutions, directives, and political activities to enhance political acumen, revolutionary outlook.
- Ethics and lifestyle: Outcomes of learning and following Ho Chi Minh's ideology, ethics, and style; maintaining ethics, lifestyle, promoting the pioneering, exemplary nature of party members, and maintaining close relationships with the populace; willingness to listen and rectify faults, as well as fighting against bureaucratic practices, corruption, wastage, negativity; commitment to building the local community, agency, unit, and maintaining internal unity.
- Discipline awareness: Adherence to organizational assignments; compliance with the rules for what party members are not allowed to do and local, agency, unit regulations; party activity principles, paying party dues according to regulations; exemplary fulfillment of civic duties and maintaining connections with the cell, local party committee at the residence; responsibility to set an example as a party member.
- Work habits and style: Proactivity, creativity, decisiveness in task execution; scientific, democratic working methods in accordance with principles; cooperative spirit, mutual assistance among comrades and colleagues.
- Fighting against manifestations of political, ethical, lifestyle decline, “self-evolution”, “self-transformation” of the individual. (personal review with reference to 27 manifestations): Self-reflect on signs such as: Unwillingness to take on distant assignments, places with difficulties; perfunctory consensus, formal democracy…

[2] Implementation of duties, powers, and results of assigned targets and tasks for the year.

Based on the following criteria:
- Fulfillment of responsibilities and powers according to the regulations (party, government, organizations) such as: Properly paying Communist Party dues;...
- Results of achieving assigned targets and tasks for the year quantified by products. For party members who are officials and public employees, it's necessary to clarify the quantity, quality, progress, effectiveness of achieving annual targets and tasks; innovation spirit, creativity, self-responsibility; service consciousness and attitude towards the citizens...
- Evaluation results, classification of organizations, agencies, units under direct leadership, management; periodic confidence evaluation results (if applicable).
- Personal responsibility related to results, limitations, and shortcomings in fields, localities, organizations, agencies, units under one’s charge: Occasionally still not focused enough on work resulting in minor errors…
[3] Implementation of annual commitments to self-improvement, training, striving
Fulfill the responsibility of being exemplary according to Communist Party regulations.
Always uphold a high sense of responsibility in task execution; innovate work methods and practices; comply with and defend, advocate, and persuade officials, party members, family, and citizens to implement the Communist Party’s directives, perspectives, strategies, resolutions, state policies, and laws.
Commit to strictly implementing Central Resolution 4, term XII, and Conclusion of Central Resolution 4, term XIII, on Party building and rectification work; no manifestations of political, ethical, lifestyle decline or “self-evolution”, “self-specialization” within the organization.
II. Limitations, shortcomings, and causes

In some work aspects, there is yet to be an appropriate arrangement of time, which leads to insufficient time for research and understanding of Macroeconomics, Regulations, Resolutions, Communist Party guidelines, laws…
III. Results of overcoming limitations and shortcomings already concluded by competent authorities or pointed out in previous assessments
Clarify each limitation, shortcoming (overcome; being addressed, extent of address; not yet addressed); difficulties, obstacles (if any); personal responsibility.

IV. Explanation of issues suggested for critique (if any)
Explain each issue suggested for critique, state the causes, identify personal responsibility for each issue suggested for critique such as:
The inspection and supervision work is sometimes not thorough, rigorous. Professional work takes up too much time.
Due to a sense of duty, the evaluation and critique work is not genuinely objective, not correctly assessing the essence of the issues.

VI. Directions and measures to overcome limitations and shortcomings
For example:
- Strive to promote strengths, address critique and self-critique feedback for colleagues, be bold and actively participate in contributing opinions more within the Party branch. In problem-solving processes, one must be flexible, proactive yet cautious, displaying determined and firm attitudes in combating and minimizing acts of embezzlement, corruption, wrongdoings among comrades and colleagues, thereby contributing to building a strong Communist Party and government;
- Regularly and actively nurture and train ethical lifestyles to enhance political and professional proficiency.
VII. Self-evaluation of quality classification
Self-evaluate quality classification into four levels:
- Outstanding task completion
- Good task completion
- Task completion
- Non-task completion
According to the provisions in clause 2, Article 10 of Regulation 124-QD/TW 2023 guiding the critique and assessment, quality classification annually for individuals within the political system:
- Criteria regarding political ideology; ethical qualities, lifestyle; discipline awareness; maintaining internal unity; adherence to organizational assignments; implementation of regulations on what party members are not allowed to do; responsibility, demeanor, habits, working methods; anti-corruption, wastage, negativity, manifestation of political, ethical lifestyle decline, "self-evolution", "self-transformation"; extent of implementation of annual commitments for self-improvement, training, striving.
- Criteria on the performance results of duties and tasks assigned annually (quantified by specific products); competence, credibility; responsibility for setting an example, the exemplary nature of oneself and family ties; maintaining close ties with the populace.
- Results of addressing, rectifying identified limitations, weaknesses, shortcomings.
Note: For individuals who are leaders, managers, and officials under the jurisdiction of the Central Committee of the Communist Party, the Politburo, the Secretariat: Evaluation is based on position standards within the Politburo's regulations and cadre management hierarchy.
