Based on Article 21 of the Regulations for lower secondary schools, Senior High Schools, and Multi-level Schools issued with Circular 32/2020/TT-BGDDT as follows:

System of Educational Activity Management Documents

The system of educational activity management documents in schools includes:

1. For the school:

a) Registration book.

b) Student report card.

c) Monitoring and assessment book for students (by class).

d) Educational documents for disabled students (if any).

đ) The school's educational plan (by academic year).

e) Classroom record book.

g) Certificate and diploma issuance management book.

h) documents and management of incoming and outgoing documents.

i) Educational equipment and library management record.

k) Asset and financial management records.

l) Staff, teacher, and employee evaluation documents.

m) Student health monitoring documents.

n) documents of student rewards and discipline.

o) Popularization education documents (for lower secondary school level).

2. For the specialized department:

a) Educational plan of the specialized department (by academic year).

b) Notes on the content of specialized departmental meetings.

3. For teachers:

a) Teacher’s educational plan (by academic year).

b) Lesson plan (teaching plan).

c) Student monitoring and assessment record.

d) Class record book (for homeroom teachers).

4. documents regulated in clauses 1, 2, and 3 of this Article in electronic form can replace paper documents following a suitable schedule based on local conditions, school capabilities, teacher implementation abilities, and ensuring the legality of electronic documents. The management of electronic documents is regulated by the Department of Education and Training based on the connectivity standards and data standards of the Ministry of Education and Training.

5. Preservation of educational activity management documents in schools is implemented according to legal regulations.

Thus, the types of documents lower secondary school teachers need to prepare for the new school year include:

- Teacher’s educational plan (by academic year).

- Lesson plan (teaching plan).

- Student monitoring and assessment record.

- Class record book (for homeroom teachers).

*Note: The aforementioned documents in electronic form can replace paper documents following a suitable schedule based on local conditions, school capabilities, teacher implementation abilities, and ensuring the legality of electronic documents.

How do subject teachers use the student monitoring and assessment record in lower secondary schools?

Based on Clause 1, Article 19 of Circular 22/2021/TT-BGDDT which regulates the responsibilities of subject teachers as follows:

Responsibilities of Subject Teachers

1. Conduct regular assessments; participate in periodic assessments as assigned by the Principal; directly record or enter grades and assessment levels into the Student Monitoring and Assessment record (by class), and the teacher's Student Monitoring and Assessment record.

2. Calculate the average subject score (for subjects assessed by both comments and numerical grades); compile assessment levels (for subjects assessed by comments) by semester and academic year; directly record or enter grades and assessment levels into the Student Monitoring and Assessment record (by class), the teacher's Student Monitoring and Assessment record, and the Student Report Card.

Thus, according to the above regulation, subject teachers have the responsibility to use the student monitoring and assessment record to directly enter grades and assessment levels in the student monitoring and assessment record by class.

2. Learning Outcomes in Each Semester and Academic Year

For subjects assessed by both comments and numerical grades, the Semester Average Grade (SAG) is used to assess student learning outcomes for each semester, and the Annual Average Grade (AAG) is used to assess student learning outcomes for the entire academic year. Student learning outcomes for each semester and the entire academic year are assessed at one of four levels: Excellent, Good, Adequate, Not Adequate.

a) Excellent Level:

- All subjects assessed by comments are evaluated as Adequate.

- All subjects assessed by both comments and numerical grades have a SAG, AAG of 6.5 points or higher, with at least six subjects having a SAG, AAG of 8.0 points or higher.

b) Good Level:

- All subjects assessed by comments are evaluated as Adequate.

- All subjects assessed by both comments and numerical grades have a SAG, AAG of 5.0 points or higher, with at least six subjects having a SAG, AAG of 6.5 points or higher.

c) Adequate Level:

- At most one subject assessed by comments is evaluated as Not Adequate.

- At least six subjects assessed by both comments and numerical grades have a SAG, AAG of 5.0 points or higher; no subject has a SAG, AAG below 3.5 points.

d) Not Adequate Level: Other cases.

3. Adjustment of Assessment Levels of Learning Outcomes

If the assessment level of learning outcomes for a semester or academic year is downgraded by two or more levels compared to the evaluation criteria specified in points a and b of Clause 2 of this Article due to the result of only one subject, the assessment level of learning outcomes for that semester or academic year is adjusted to the next higher level.

Accordingly, the four levels of assessment of student learning outcomes in lower secondary schools according to the regulations are: Excellent, Good, Adequate, Not Adequate.
