What is the sample Leave Application Form for students in Vietnam?

The new academic year 2024-2025 has begun according to Decision 2045/QD-BGDDT in 2024, issued by the Ministry of Education and Training. It establishes the academic calendar for the 2024 - 2025 school year for preschools, general education, and continuing education nationwide, with school opening ceremonies held on September 5, 2024, at most schools across the country.

Students, including those in primary, lower secondary, and upper secondary schools, who need to take day off due to important matters or incidents, can refer to the latest leave application form and guidelines on how to write it:

Detailed instructions on how to write Leave Application Form for students in Vietnam:

- National title and motto: (Should be written carefully and completely, paying attention to capitalized words)

"Socialist Republic of Vietnam

Independence - Freedom - Happiness"

- Location, date ... month ... year

(Usually located on the right side where the student fills in the date they start writing the application; in case of permission for a day already taken off, the date the application is written is also noted here).

- LEAVE APPLICATION FORM

(The title of the form must be written in all capital letters)

- To:

(This part shows to whom the application is addressed to request permission. The student fills in this section in order:

+ Principal of ... school

+ Homeroom teacher of class …………………...

+ Subject teachers of the class

Example: - To: The Board of Principals and the homeroom teacher of class....

- My name is: Write the full name of the student (In capital letters)

- I am a student of class: Clearly state the current class of the student.

- Leave duration: I write this application to request permission from the teacher to let me take leave on the day:

(Specify the date on which the student requests leave, or if taking permission for a past day, write the date already taken off).

- Reason:

State the reason for requesting leave, e.g., illness, important family matters, accident, hospital admission for monitoring, etc. If the leave is long, provide supporting documents from the hospital.

In this case, students should provide as specific a reason as possible. Avoid giving vague reasons like family matters, going to a wedding. which may not get approved.

- Commitment and signature of the student and confirmation from parents regarding the student's leave.

Example:

+ After the leave, I promise to take notes and study adequately. If I lie or fail to study, I will accept any punishment from the school and the teacher. I hope for your approval.

+ I promise to copy notes, study, and complete assignments properly. Kindly consider allowing me this day off.

- Finally, a sincere thank-you note.

+ I sincerely thank the teachers for approving my leave.

- Parental confirmation: Parents sign and provide their full names.

Based on Article 12 of the Regulation on evaluation and grading of secondary and upper secondary school students issued with Circular 22/2021/TT-BGDDT:

Promotion, summer re-evaluation, non-promotion

1. Students who meet the following conditions will be promoted or recognized as completing the lower secondary or upper secondary program:

a) The conduct evaluation for the entire school year (including re-evaluation after summer re-training as per Article 13 of this Circular) meets the minimum of "Pass."

b) The academic performance for the entire school year (including the re-evaluation of subjects as stipulated in Article 14 of this Circular) meets the minimum of "Pass."

c) The student does not miss more than 45 school days in one academic year (based on the one-session/day educational plan stipulated in the General Educational Program, including both approved and unapproved leave, continuous or non-continuous absences).

2. Students who need to undergo re-training during the summer must comply with the provisions of Article 13 of this Circular; students who need to be re-evaluated in subjects must follow the provisions of Article 14 of this Circular.

3. Students who do not meet the requirements specified in clause 1 of this Article will not be promoted or recognized as completing the lower secondary or upper secondary program.

4. For students with disabilities: Based on the evaluation results of disabled students as prescribed in Article 11 of this Circular, the principal decides whether to promote or recognize the completion of the lower secondary or upper secondary program for disabled students.

Thus, students are allowed a maximum of 45 days off per academic year with permission. If they exceed this limit, they will not be promoted to next grade.

Do students in Vietnam need to join make-up classes after authorized leave?

Based on Article 12 of the Regulation on evaluation and grading of lower secondary and upper secondary school students issued with Circular 22/2021/TT-BGDDT:

According to the regulations, there is no explicit requirement that students must join make-up classes after authorized leave.

However, students have committed to supplementing notes and completing assignments after taking authorized leave in their application form.

Therefore, it is the student's responsibility to actively fulfill this commitment after the leave by borrowing notes from classmates and asking them about the lessons missed.

Further proactive measures include seeking out lectures from other teachers online to reinforce their knowledge.
