What is the format of the study record of primary school students in Vietnam?
Pursuant to Appendix 1 promulgated with Circular 27/2020/TT-BGDDT which regulates the assessment of primary school students issued by the Ministry of Education and Training, the format of the study record of primary school students is as follows:

How to fill out the study record of primary school students in Vietnam?
Based on the content in Appendix 1 promulgated with Circular 27/2020/TT-BGDDT, the study record is used to record the summary results of the end-of-year assessment of students. When recording the study record, teachers need to carefully study Circular 27/2020/TT-BGDDT concerning the regulations on the assessment of primary school students.
The instructions for filling out the study record are as follows:
(1) Page 3, information recorded according to the student's birth certificate.
(2) Section "1. Subjects and Educational Activities"
- In the column "Achieved Level": Record the symbol T if the student achieves the level "Excellent"; H if the student achieves the level "Good" or C if the student is at the level "Not Yet Achieved".
- In the column "Periodic Check Score" for subjects with Periodic Test: record the score of the end-of-year test; for students who retake the test, record the score of the last test.
- In the "Comments" column: Record notable points about the student's progress, talents, and interest in studying subjects, educational activities. Note any content, skills not yet completed in each subject, and educational activity that needs improvement and assistance (if any).
(3) Section "2. Key Qualities" and section "3. Core Competencies"
- In the column “Achieved Level” corresponding to each assessment content on quality, competency: Record the symbol T if the student achieves the level “Good”, D if the student achieves the level “Achieved” or C if the student is at the level “Needs Improvement”.
- In the "Comments" column corresponding to the assessment content on quality: Record the expressions, progress, strengths, limitations, or recommendations (if any) regarding the formation and development of some key qualities of the student.
Example: Attending school on time; boldly presenting personal opinions; keeping promises; respecting and helping others; ...
- In the "Comments" column corresponding to the assessment content on competency: Record the expressions, progress, strengths, limitations, or recommendations (if any) regarding the formation and development of some general and specific competencies of the student.
Example: Practicing good personal hygiene, dressing neatly; being proactive, cooperating in learning; having self-study ability; ...; using language fluently in life and learning, thinking, reasoning and solving some familiar math problems; ...
(4) Section "4. assessment of Educational Outcomes"
Record one of four levels: “Outstanding Completion”; “Good Completion”; “Completion” or “Not Yet Completed”.
(5) Section "5. Commendation"
Record the achievements the student has been commended for during the school year.
Example: Achieved the title of Excellent Student; Achieved the title of Exemplary Student who completed well in study and training; Won the Second Prize in the District Traffic Safety Exchange; ...
(6) Section "6. Completion of Class Program/Primary School Program"
Record "Completed the program of class ......../ primary school program" or "Not yet completed the program of class ......./ primary school program"; "Promoted to the next grade" or "Not promoted".
Example:
- Completed the program of class 2; Promoted to class 3.
- Completed the primary school program.
The study record is managed by the school and returned to the student when the student transfers schools or completes the primary school program.

Pursuant to Clause 2, Article 9 of the Regulations on the assessment of primary school students promulgated with Circular 27/2020/TT-BGDDT, it is regulated as follows:

2. By the end of the school year, based on the summary of the assessment results of each subject, educational activity, and each key quality, core competency, the homeroom teacher shall perform:
a) Evaluate the educational results of students according to four levels:
- Outstanding Completion: Students with assessment results of subjects, educational activities at Good level; qualities and competencies at Good level; periodic final exams with scores of 9 or higher;
- Good Completion: Students not in the Outstanding Completion category, but with assessment results of subjects, educational activities at Good level; qualities and competencies at Good level; periodic final exams with scores of 7 or higher;
- Completion: Students not in the Outstanding or Good Completion categories, but having assessment results of subjects, educational activities at Good or Satisfactory levels; qualities and competencies at Good or Satisfactory levels; periodic final exams with scores of 5 or higher;
- Not Yet Completed: Students not in the above categories.
