Template of health examination form for enrollment in the academic year 2024 in Vietnam
Based on Template No. 1, Appendix XXIV, the health examination form used for individuals aged 18 and above, issued together with Circular 32/2023/TT-BYT, the form will be as follows:

Are health examinations required for students upon enrollment in Vietnam?
Based on the regulations in Article 30 of Circular 32/2023/TT-BYT, the subjects required to undergo health examinations include:
Health examination Subjects
1. Health examination subjects:
a) Health examinations for Vietnamese citizens and foreigners living and working in Vietnam: regular health examinations; health classification checks for study or work; health examinations for students and college students;
b) Health examinations upon request;
c) Health examinations for Vietnamese individuals going abroad for work under contracts.
2. Health examinations as outlined in this Chapter do not apply to the following cases:
a) Outpatient and inpatient checks within medical examination and treatment facilities;
b) Forensic medical and psychiatric forensic assessments;
c) Checks for issuing injury certificates;
d) Occupational health examinations;
e) ealth examinations for subjects under the jurisdiction of the Ministry of Defense and Ministry of Public Security;
f) Health examinations for specific sectors and professions.
Thus, according to the above regulation, students are among the subjects required to undergo health examinations.
What are the procedures for health examinations for students in Vietnam?
In addition, the procedures for health examinations will be based on the regulations in Article 35 of Circular 32/2023/TT-BYT as follows:
- Individuals seeking health examinations must submit their documents in the prescribed format at the health examination facilities.
- Upon receiving the health examination documents, the health examination facility will perform the following:
+ Compare the photograph in the health examination documents with the individual seeking the health examination;
+ Seal the photograph after verification as specified at point a of this section for cases stipulated in Clauses 1, 2, and Clause 3 of Article 37 Circular 32/2023/TT-BYT;
+ Validate legal identification documents of the patient's relatives for cases stipulated in Clause 3 of Article 37 Circular 32/2023/TT-BYT;
+ Guide the health examination process of the unit for the individual undergoing the health examination or the guardian of the individual undergoing the health examination (if any);
+ health examination facilities develop, issue, and implement health examination procedures;
+ Provide results and retain health examination records.
Is it mandatory for students in Vietnam to participate in health insurance?
Based on Article 4 of Decree 146/2018/ND-CP (amended by points a and b, Clause 2, Article 1 of Decree 75/2023/ND-CP) on the subjects required to participate in health insurance:
Group supported by the state budget
1. Individuals from near-poor households according to the near-poor household standards for 2022-2025 specified in Decree No. 07/2021/ND-CP and other documents issued by competent authorities amending, supplementing, or replacing the near-poor household standards applicable for each period.
2. Individuals from multidimensionally poor households not covered under point a, Clause 9, Article 3 of this Decree.
3. Students.
4. Individuals from households engaged in agriculture, forestry, fishery, and salt production with average living standards according to the average living standards household norms for 2022-2025 specified in Decree No. 07/2021/ND-CP and other documents issued by competent authorities amending, supplementing, or replacing the average living standards household norms applicable for each period.
Additionally, based on point a, Clause 5, Article 9 of Decree 146/2018/ND-CP:

5. For students as stipulated in Clause 3, Article 4 of this Decree:
a) Every 3 months, 6 months, or 12 months, students or their parents or guardians are responsible for paying the health insurance contribution as stipulated in Clause 2, Article 10 of this Decree to the social insurance agency;

Therefore, in accordance with the above regulations, students are required to participate in health insurance. Additionally, students will be in the group supported by the state budget.
Moreover, every 3 months, 6 months, or 12 months, students or their parents or guardians are responsible for paying the designated portion of health insurance contributions to the social insurance agency.
