The current teacher's personal achievement report template is the achievement report template for commendation - Template No. 03 Appendix I issued together with Decree 98/2023/ND-CP.

Note: Template No. 03 is used to report achievements proposing to award or posthumously award Orders, the Diploma of Merit of the Prime Minister of Vietnam, National Emulation Fighters; Emulation Fighters, Diplomas of Merit of ministries, departments, and central or local authorities, and Certificate of merit for individuals with achievements in performing assigned tasks.
Below is the guidance on preparing the teacher's personal achievement report in Vietnam under Decree 98:
I. Personal Background
Teachers should fill in all personal information, including:
- Full name, date of birth;
- Gender, ethnicity, religion;
- Hometown, permanent residence;
- Affiliated institution, local workplace;
- Position: (specify the subject taught, if the teacher is a manager, state the managerial title, e.g., Head of the professional team of Mathematics;
- Level of political theory, specialization, profession (if any);
- Academic rank, degree, titles, awards.
II. Achievements Obtained
1. Granted Rights and Responsibilities:
Teachers need to specifically list the tasks and main responsibilities that they have taken on in their current position.
Teachers should clearly state what subjects they teach, classes they are responsible for, subjects they supervise, extracurricular activities, and other tasks assigned by the school or organization.
Specific Example:
- Teaching Mathematics for grades 7 and 9.
- Serving as the homeroom teacher for class 9A for three consecutive years.
- Acting as the Head of a professional team of Mathematics, responsible for creating teaching plans, supporting colleagues, and organizing professional development sessions.
- Guiding students to achieve high results in math competitions.
- Actively participating in emulation movements, group activities, and social work.
- Contributing to the development of the thematic “Innovating Teaching Methods.”
2. Individual Achievements:
Teachers should clearly state personal results achieved through each task. Some specific suggestions:
a. Results in productivity, quality, and work efficiency:
- List student achievements in excellent student contests, art, and sports competitions.
- Rate of student improvement and high performance over the years.
- Innovations and initiatives in teaching.
Specific Example:
- Guided students to win 5 first prizes, 3 second prizes in the County Excellent Student Contest for the 2023-2024 academic year.
- Increased the proportion of students achieving Excellent academic performance from 70% to 85% over the past two years.
- Proposed and successfully implemented the teaching method integrating information technology, highly appreciated by the professional team.
b. Initiatives, scientific research projects
If teachers have implemented initiatives or scientific research projects, describe briefly the content, results, and application value.
Specific Example:
- The initiative “Applying Mind Maps in Literature Teaching” was recognized at the district level in 2023.
- The research project “Enhancing Writing Skills for Grade 9 Students” helped improve the average score in Literature by 30%.
c. Professional development and study activities
Courses, workshops participated in, and results achieved.
Specific Example:
Participated in an advanced teaching professionalism course at the Department of Education and Training in 2022, and achieved an Excellent certificate.
d. CPV-building and organizational work
- Actively participated in CPV, HSM Communist Youth Union, and trade union movements within the school.
- Set an exemplary role in work and life.
Specific Example:
Participated in the charity program “Donating Books to Students in Remote Areas” as an organizer, raising over 500 books.
III. Emulation Titles, Awards Received
- Teachers list the types of awards granted by the Communist Party of Vietnam, the State, ministries, departments, and central or local authorities (specify the decision number, and date of signing the decision).
- Clearly state decision numbers recognizing emulation titles within 5 years before proposing for Labor Order, Diploma of Merit of the Prime Minister of Vietnam, etc.
- For the recommendation of the “National Emulation Fighter” title:
+ Clearly state the decision number, date of signing the decision of 2 consecutive times receiving the Emulation Fighter title from ministries, departments, and central or local authorities, and 6 times receiving the “Grassroots-Level Emulation Fighter” title before proposing;
+ Clearly state the initiatives or scientific research topics, scientific projects, scientific and technological works (participated as a member or project leader), with certification or acknowledgment from authorized agencies or organizations (specify the document, date) regarding initiatives, scientific research topics, scientific projects, scientific and technological works accompanying the dossier.
- For the report proposing the “Emulation Fighter of ministries, departments, and central or local authorities” title, clearly state the time of 3 consecutive times receiving the “Grassroots-Level Emulation Fighter” title before proposing; the initiatives or scientific research topics, scientific projects, scientific and technological works recognized by competent authorities.

What are the emulation titles for teachers in Vietnam?
According to Article 19 of the Law on Emulation and Commendation 2022, the emulation titles for individuals, including teachers, consist of:
- “National Emulation Fighter”.
- Emulation Fighter of the Ministry, Department, and Central or Provincial Authority.

- One commendation form may be awarded many times to one subject; prohibition against provision of multiple awards or other forms of commendation to one achievement; provision of commendation to the corresponding achievement;
- Focus on the provision of commendations to individuals, collectives, and households that directly work, produce, and conduct business; individuals and collectives that work in borders, seas, islands, and regions with extremely difficult socio-economic conditions.
