On May 15, 2021, the Ministry of Public Security issued Circular 56/2021/TT-BCA on forms for residence registration and management in Vietnam.
New: 16 forms used in residence registration and management as of July 1, 2021 in Vietnam

Issued together with this Circular are 16 forms used in registration and residence management, denoted CT, specifically as follows:
1. The declaration of change of residence information is used when citizens carry out procedures for permanent residence registration, deletion of permanent residence registration, household separation, adjustment of information in the residence database, procedures for registering temporary residence, deleting temporary residence registration, extending temporary residence, declaring residence information, and confirming residence information (symbolized as CT01).
2. Application form for issuance of written consent for permanent residence registration to be used when Vietnamese people residing abroad return to Vietnam for permanent residence and carry out the procedures to request a written consent for permanent residence registration at the Immigration and Checkpoints Authority (symbol CT02).
3. Temporary absence declaration form is issued to citizens to declare temporary absence according to the provisions of Clause 1 and Article 31 of the Law on Residence (symbol CT03).
4. The application receipt form and appointment to return results are prepared by the residence registration agency to handle administrative procedures related to residence (symbolized as CT04).
5. Instructions for supplementing and completing documents are prepared by the residence registration agency to request citizens to supplement and complete permanent residence registration documents, delete permanent residence registration, separate households, and adjust residence information in the Residence Database, including confirmation of residence information, temporary residence registration, deletion of temporary residence registration, extension of temporary residence, declaration of temporary absence, and declaration of residence information (denoted as CT05).
6. A refusal form to receive and process documents is prepared by the residence registration agency to notify citizens of the reason for refusing to receive, process permanent residence registration documents, delete permanent residence registration, separate households, adjust residence information in the residence database, confirmation of residence information, temporary residence registration, deletion of temporary residence registration, extension of temporary residence, declaration of temporary absence, and declaration of residence information (denoted as CT06).
7. Confirmation of residence information is used to confirm information about time, location, form of residence registration, and other contents of individuals and households upon request (symbol CT07).
8. Notice of results of settlement and cancellation of residence registration shall be made by the residence registration agency to notify citizens about the results of permanent residence registration, deletion of permanent residence registration, separation of households, adjustment of residence information in the residence database, confirm residence information, temporary residence registration, temporary residence registration deletion, temporary residence extension, declare temporary absence, declare residence information, cancel permanent residence registration results, and cancel temporary residence registration results (symbolized as CT08).
9. The decision to cancel residence registration is used by competent authorities to cancel permanent residence registration or temporary residence registration of citizens who do not have the right authority, the wrong subjects, or the wrong conditions according to the provisions of Article 35 of the Law on Residence (symbolized as CT09).
10. Residence information verification form is prepared by the residence registration agency to exchange information and documents between residence registration agencies to clarify information about individuals and households during the process of registration and residence management (symbolized as CT10).
11. Application approval form is used to propose to the competent authority to resolve permanent residence registration, delete permanent residence registration, separate households, and adjust residence information in the residence database, including confirmation of residence information, temporary residence registration, deletion of temporary residence registration, extension of temporary residence, declaration of temporary absence, declaration of residence information, cancellation of permanent residence registration, and cancellation of temporary residence registration (symbolized as CT11).
12. Statistics on the situation and results of registration and residence management are used by police units and localities to compile and report information related to registration and residence management in the locality, direction by day, week, month, quarter, 6 months, and year (symbolized as CT12).
13. Residence file bag is used to store records and documents in residence registration and management (symbolized as CT13).
14. The residence reception book is used to monitor the reception of residence notification contents according to the provisions of Article 30 of the Residence Law (symbolized as CT14).
15. Book to monitor delivery and receipt of residence documents created and managed by the agency managing residence records archives after resolving permanent residence registration, deleting permanent residence registration, separating households, adjusting residence information in the residence database, and canceling permanent residence registration results (denoted as CT15).
16. Book to monitor and exploit the residence records archive created by the agency managing the residence records archive and managing information and documents in the residence records archive (symbol CT16).
Instructions for using the form
- Forms CT01, CT02, CT03, CT04, CT05, CT06, CT07, CT08, CT09, CT10, CT11, and CT12 are integrated into the residence management software and printed for use. For forms with numbers, the number of that form is recorded by the residence registration agency in ascending order, starting from number 01 on the first day of the year and ending on December 31 of every year.
- Forms CT14, CT15, and CT16 must be written consecutively in order on each page and must not be left blank; Must write fully and clearly the contents of the cover and pages of the notebook.
Note: It is strictly forbidden to arbitrarily erase, correct, or add to the information content in the form. In cases of intentionally falsifying information, depending on the severity, it will be handled according to the provisions of the law.
Circular 56/2021/TT-BCA takes effect on July 1, 2021, and replaces Circular 36/2014/TT-BCA.
