Circular 35/2015/TT-BCA stipulates that the sample Commendation Certificate in the People’s Public Security must adhere to the following form and content:

Form: rectangular shape, size equivalent to A4 paper, length 297mm, width 210mm, the outer dimension of the decorative border length 282mm, width 175mm; white paper weighing 150 grams/m2. The border is decorated with floral patterns (different from the floral patterns used in the State-level commendation forms, Ministerial Commendation, and Commendation from the General Department, Mobile Police Command), with the National Emblem of the Socialist Republic of Vietnam centered at the top of the border; with no patterns in the four corners outside the border, and the transition pattern between the National Emblem and the decorative border. The background is plain white.

Content:

- The first line states the country name: “SOCIALIST REPUBLIC OF VIETNAM” presented in uppercase letters, font “Times New Roman”, size 14, upright, bold, black color;- The second line states the motto: “Independence - Freedom - Happiness” presented in lowercase, font “Times New Roman”, size 14, upright, bold, black color, immediately below the country name, the initial letters of the phrases capitalized, hyphenated between phrases, spaced between phrases, with a horizontal line underneath, solid, the length equal to the length of the line of text;- The third line states the title of the person authorized to award the Commendation Certificate, presented in uppercase, font “Times New Roman”, size 18, upright, bold, red color;- The fourth line states the word “AWARD” presented in uppercase, font “Times New Roman”, size 16, upright, bold, black color;- The fifth line states the phrase “COMMENDATION CERTIFICATE” in uppercase, font “Times New Roman”, size 14, upright, bold, red color;- The sixth line states the name of the individual awarded the commendation in uppercase, font “Times New Roman”, size adjusted accordingly, upright, bold, black color;- The seventh line states the information of the collective, individual awarded the commendation (title, workplace), presented in lowercase, font “Times New Roman”, size adjusted accordingly, upright, bold, black color;- The eighth line states the commendation text in lowercase, font “Times New Roman”, size adjusted accordingly, italic, bold, black color;- Bottom left: the first line states “Decision number, date, month, year” (written in numbers, date, month, year of the decision issuance); the second line states “Golden Book number” (written in sequential order of the recipient of the Commendation Certificate in the decision), the text of both lines in lowercase, font “Times New Roman”, size 12, italic, bold, black color;- Bottom right: the first line states the location, date, month, year in lowercase, font “Times New Roman”, size 13, italic, bold, black color; the second line states the title of the person authorized to award the Commendation Certificate in uppercase, font “Times New Roman”, size 13, upright, bold, black color; space for signature and seal; the last line states the rank, name of the person authorized to decide to award the Commendation Certificate.
