Dossier for Nomination for the Title of People's Doctor, Excellent Doctor Includes What?

Decree 41/2015/ND-CP stipulates the dossier for the application for the title of "People's Doctor" and "Eminent Doctor" as follows:

1. Individual application dossier includes:

- Achievement declaration form for the title of "People's Doctor" according to Form No. 08, "Eminent Doctor" according to Form No. 09 stipulated in the Appendix issued together with this Decree;

- Documents proving achievements and contributions in professional activities, scientific research, and contributions to the cause of protecting, caring for, and improving people's health, including: Certified copies if the individual submits the dossier by post or photocopies accompanied by the original for comparison if submitting directly at the Standing Body of the grassroots-level council for certificates or decisions awarding orders, medals, prizes, honorable State titles, and other forms of competition and commendation; recognized results of the Council for Science and Technology, the competent authority's initiative council, or patent right, innovation certificate.

2. The application dossier from the lower-level council to the higher-level council includes:

- Proposal report for the title of "People's Doctor," "Eminent Doctor" according to Form No. 01 stipulated in the Appendix issued together with this Decree;- List of proposed titles for "People's Doctor," "Eminent Doctor" according to Form No. 02 stipulated in the Appendix issued together with this Decree;- Minutes of introduction and results of opinion collection according to Form No. 03 stipulated in the Appendix issued together with this Decree;- Minutes and results of the approval vote according to Form No. 04 stipulated in the Appendix issued together with this Decree;- Summary list of individual achievements proposed for the title of "People's Doctor" according to Form No. 05 stipulated in the Appendix issued together with this Decree;- Summary list of individual achievements proposed for the title of "Eminent Doctor" according to Form No. 06 stipulated in the Appendix issued together with this Decree;- Report on the process of awarding the title of "People's Doctor," "Eminent Doctor" according to Form No. 07 stipulated in the Appendix issued together with this Decree;- Decision on the establishment of the Council;- Individual dossier as stipulated in Clause 1 of this Article.
